Fill before Your First Visit:

On your first visit we will need to collect a lot of information about your medical history, your symptoms, your general state of health and wellbeing. We also need you to read and sign some paperwork. It will make it easier if you come prepared.

Please review the Forms in our Self-Service Desk. It will be great if you could fill and print them and bring them to our first meeting.
